This young lady came in after 2 pregnancies followed by a gastric sleeve weight loss operation and loss of 120 pounds!  These days we see more and more plastic surgery candidates who have lost massive amounts of weight due to either diet & exercise and/or weight loss surgery.  This is not uncommon, but surgery is different than straight forward breast lifts.  Tissue that has been subjected to such severe weight loss often loses not only its elasticity but the anchorage points which fix the tissue to the skeletal frame are often stretched and lax.  Most of these patients will not do with just breast implants alone and need a mastopexy (breast lift) in order to replace the areola back at an appropriate level on the chest wall and remove loose skin from the bottom of the breast.  This patient only has a moderate amount of ptosis (sag) despite her extreme weight loss and was benefited by a vertical (lollipop) mastopexy with a silicone 475cc implant.  She is shown before and 2 months after her surgery.  Scars will take approximately a year to mature.
