This is a case of a patient who came to see me after multiple surgeries performed somewhere else including breast augmentation, breast augmentation revision, and scar revision as well as fat transfer.  Her current implants were saline implants as she went to a surgeon who "does not like silicone implants" and thus continued to revise her saline even though a switch to silicone would dramatically improve her results.  5 years ago he revised her with larger saline implants and when this caused more (not less) rippling as would be expected he then suggested fat grafting 3 years ago to hide the rippling.  The fat grafting made minimal improvement so she now came to see me for revision.  When you look at her preoperative photographs you can see the large amount of rippling.  The postoperative photographs still have a very small amount of rippling but it is decreased by 90%.  If I were doing this case in 2017 I would use Allergan's newer highly cohesive Inspira Cohesive breast implant as it even reduces rippling more.  She is shown here before surgery and after replacement of her 475cc high profile saline implants with 650cc full profile implants as well as pocket modifications including internal reshaping.  She is very pleased with the improvement of her breasts.
