This is a 37 year old patient who came to see me after pregnancy and weight loss concerned with breast deflation and loss of volume.  She has a bit of laxity to her skin with some mild drooping and I recommended that the best option would be to do a mastopexy or breast lift to remove and reposition loose skin.  She did not want to do this because of the scarring involved and as she was borderline for "needing" the lift I agreed to place implants without a lift.  This is always a difficult discussion because as a plastic surgeon I want to get the best possible result for every patient and yet some patients are borderline and can still get a very nice result without a lift even though there may still be a little less perkiness than ideal.  This is a tradeoff for scars which some patients accept.  There are other patients who simply are droopy enough that the absolutely need a lift and I will not just place implants as they won't get a good result at all.  I placed 415cc Sientra gummy bear cohesive silicone breast implants in a dual plane position and she is shown here at 6 weeks with a very nice natural result.
