This lovely patient initially came in requesting fat transfer to her breasts as she did not necessarily want foreign bodies like implants in her body.  This is a common request and fat transfer is certainly a reasonable option but patients need to understand the limitations.  First of all fat needs to be given a chance to take up a blood supply in order to survive and so only limited amounts can be placed.  At maximum 150-200cc can be successfully transferred and often one side will take more than the other and this will require more surgeries to equalize everything.  I explain to patients that it often takes two to three surgeries with fat grafting and they need to know this going into it in order to plan on recoveries and expenses involved.  Yes implants are foreign materials and have their own risks.  I explain to all patients that breast implants do not last forever.  Newer Motiva breast implants have a lower risk of rupture at 10 years but there are other reasons besides leaks that ladies require revisions.  Capsular contracture while quoted at 1% with Motiva breast implants is real.  Breasts droop over time.  Bodies change as we age around the implants and sagging can occur.  Weight changes occur and the implant stays the same as when it was placed.  Thus many ladies that get implants will ultimately need revisions and this needs to be understood.  This patient wanted natural looking implants and chose Motiva's Ergonomic implant as they look more natural with less upper pole fullness.  She is shown here before and after subfascial placement of 425cc ergonomic Demi profile breast implants via crease incisions.  Tattoos are removed for privacy.
